Becoming a Patient

It is easy to become a new patient at the STAR Health Center (SHC). In this section, you can find out about the registration process and the steps involved becoming a SHC patient. For any questions, please call us at 718.270.3745.

 Appointments

The SHC has both daytime and evening appointments available for all medical and mental health services. We want to help you maintain your health, and we will remind you of your appointments via phone call or text, whichever is best for you.

 Medical Records

Your privacy is important to us and we will protect your personal health information. See this section for information on our privacy practices and how to go about requesting or sending your medical records to or from other health care providers.

Patient Rights and Responsibilities

At SHC, our relationship with our patients is a partnership. This section describes your rights as a SHC patient as well as your responsibilities to ensure that you and your fellow patients receive excellent care in a comfortable, pleasant environment.

Health Care Proxy

A health care proxy is the best way to ensure that someone you trust has the right to make decisions about hour care in the event you are unable to make these decisions yourself.   See this section for information and forms.

Insurance

The SHC accepts most insurance plans. If you do not have insurance, our case management team will work with you to sign up for insurance, Medicaid or special programs to help you pay for your visits and medications. No one will be denied access to services due to inability to pay, and there is a discounted/sliding fee schedule available based on family size and income.
